汽車入油月結咭(申請及信用卡)聲明

    本人茲申請開戶及發予加友易入油記賬咭“記賬咭”，並保證上述資料全部正 確無誤，及授權 貴公司取得及交換有關本人之信貸資料。本人定必依照 貴公 司規定於月結單內所訂明之繳費日或之前繳清全部款項。本人明白使用記賬咭 的條款及細則，將詳列於隨咭附上的文件內，並明白使用該咭將被視為接受該 條款及細則。本人同意，本人所提供的資料可構成澳門特別行政區法律“個人 資料(私穩)條例”所定義之“個人資料”。通過交還本申請表，本人同意加友 易可使用或向任何第三者透露此申請表中所提供的資料及有關本人的賬戶資料 以用於推廣營銷、管理、信貸調查或收取賬款之目的。本人保證承擔上列登記 之車輛經加友易申請蜆殼之油咭，到蜆殼油站購買各類石油產品之貨款。
I hereby warrant that the above information is true and correct and request to open an account with the respective Friendly Service card(s)to be issued to me. I undertake to settle the account on or before the due date via pre-approved credit card as shown in the statement of account. I authorize you to exchange and receive credit information pertaining to me. I note that the Terms and Conditions of the Friendly Service card will be set out in the package delivering the Friendly Service card to me. I further agree that my use of the card will mean my acceptance of the terms and conditions. I accept that the information which I supply may constitute Personal Data as defined in the Personal Data (Privacy)Ordinance, Law of Macau SAR. By returning this application form, I agree that information provided in this application and information about my account may be used or disclosed by Friendly Service Limited to any third party for promotion marketing, administrative, credit checking or account collection purposes. I take full liabilities for the settlement of payment for the purchases of all petroleum products applied via authorized card agent Friendly Service Limited from Shell filling stations in Macau by the fuel cards.

Welcome to use a valid credit card issued by Macau or Hong Kong bank. The invoice amount to be charged for fuel consumption is settled in MOP and Macau respectively. Highly recommend to use credit card issued by Macau bank, otherwise credit card purchase may incur oversea transaction fees

     本人授權加友易服務有限公司從本人之信用卡賬戶內扣除賬款繳付上述申請表上登記之車輛 的應付費用，並知悉及遵守有關信用卡條款及細則，若以上資料有變更，本人必須以書面通 知貴司方可繼續生效，本人明白如遺失該信用卡，以上之自動繳費服務將於新卡發出前失效 ，加友易服務有限公司有權暫停油咭運作，直至提交新咭資料。
I hereby authorize Friendly Service Limited to debit my credit card account to settle the payment for the vehicle(s) listed in this application form. I also hereby understand and agree to the terms and conditions of credit card. Any changes of the above information, I should notify your company in writing to remain this service in force. I understand that if the credit card was lost, this Credit Card Transfer Payment Service will be invalid until the issuance of the new card, and Friendly Service Limited reserves the rights to suspend the fuel card services until the new card information is submitted.

     如該款項未能自本人之信用卡賬戶內支付，貴司有權追討本人為上述申請表上登記之車輛應 付的費用
If the payment cannot be debited from my credit card account, your company will be entitled to pursue the payment for the vehicle(s) listed in this application form.

     如有任何使授權書失效之變更，本人必須於扣賬日前10天以書面通知貴司，貴司在收到書面 通知前，本授權書繼續有效。
Any changes of this authorization should be given by written notice at least 10 days prior to the date on which the debit is made to my credit card account. This authorization shall be remain in force unless such notice is received by your company.
